SCHEDULING

Appointments are held in my office, and are generally available:

1. •Monday: 9:00am-5:00pm
   

2. •Tuesdays-Thursdays: 9:00am-9:00pm
   

3. •Mondays: 9:00am-5:00pm
   

4. •Saturdays: Based on availability; additional fee applies
   

5. •Sunday: Closed
   

PRICES

1. •Assessments: $135
   

2. •Regular appointments: $90
   

3. •Saturday Fee: Regular price + 20% fee
   

PAYMENT OPTIONS

I accept cash, personal checks, and Visa & MasterCard (debit or credit). You can pay monthly or per session.

Using Medical Insurance

Please note that I do not accept medical insurance directly. Instead, if you wish to utilize your insurance, I will take payment directly from you and then provide you with receipts to give to your insurance company to seek reimbursement. Whether this is an option for you depends entirely on your coverage. For details, please contact your insurance company and ask them to tell you about your out-of-network benefits for counseling and/or psychotherapy. I will do whatever I reasonably can to help you utilize your insurance benefits whenever possible.

Ecclesiastical Assistance for LDS Church Members

If you are unable to utilize insurance and you are unable to afford the full fee out of pocket, your Bishop may decide to utilize Fast Offering funds to help cover all or a portion of your fees. If so, I will gladly bill your Bishop monthly and stay in regular contact with him regarding your progress. Please discuss this option with your Bishop before scheduling an appointment as it is entirely up to his discretion. Please note that Bishops are charged the same amounts for services as they are charged through LDS Family Services.
